Abstract: CommonWell Health Alliance, a not-for-profit trade association, is dedicated to the simple vision that patient data should be available regardless of where past care has occurred. Following this vision for the past 10+ years, the Alliance has built a large-scale network where millions of clinical documents are exchanged daily. Learn more about how this works efficiently and effectively by using a combination of a Master Person Index (MPI) and a Record Locator Service (RLS) with support for external frameworks like Carequality and TEFCA to achieve scalable interoperability at the national level.

Linda PattersonUse Case: Linda Patterson is a 66-year-old retiree who is looking forward to enjoying years of travel and relaxation. She has mild hypertension that historically has been controlled, but recently has experienced shortness of breath. She sees a cardiologist who orders an electrocardiogram and echocardiogram which show abnormal results and subsequently fails a stress test. She undergoes cardiac catheterization which reveals multiple blockages in the coronary arteries. Her cardiologist advises that she is a candidate for coronary artery bypass grafting (CABG) and after discussing the benefits, they proceed with the surgery. Upon successful completion of the surgery and an inpatient stay, the patient is discharged home and scheduled for follow-up care in the cardiologist’s office.

Linda’s providers leveraged their EHRs to share her clinical records at each stop along her health care journey. By using the CommonWell Master Person Index (MPI), Record Locator Service (RLS), and Data Broker, Linda’s care team can see all her records when and where they need it. In addition, Linda herself can use an application on her phone to access her own records, using Individual Access Services (IAS). Thanks to the connected providers and products using the CommonWell network, Linda receives streamlined healthcare and avoided gaps in care.
