This week, many of our members are ramping up for a presence at HIMSS22. That includes those involved in our Interoperability Showcase: “Nationwide Connected Care Use Case Demo.” This demo shares with the industry how far CommonWell has come and how its reach has grown by following a fictional patient through some very real-life health challenges, coordinating care every step of the way.

The Garage

The Garage provides a comprehensive population health management platform where Providers, Care Managers, Patients and Physicians collaborate more effectively for improved patient care and lower costs. The Garage has impacted more than 15 million patient lives by partnering with more than 130 healthcare organizations (including 40+ ACO’s and DCEs). Its Population Health Management Platform, BRIDGE, has enabled over 18,000 providers across 32 states to achieve the Quadruple AIM of lower cost, better care, improved health and improved work life of healthcare providers.


Truentity Health

Truentity Health’s mission is to help reduce medication-related errors and optimize medication therapies for providers, pharmacies and hospital systems. Before and after a patient’s hospital visit, a licensed pharmacist conducts a comprehensive medication reconciliation (medrec) process to compile a complete record of their prescription medications, over-the-counter (OTC) drugs, supplements, drug allergies and past adverse reactions. This record is then securely transmitted over the Truentity HealthX network to the hospital system’s EHR, giving the provider a holistic view of the patient’s medication history to help reduce errors and optimize therapies.
